I use our hospital's patient access network to communicate with my transplant team and pre-read my test results to insure I'm asking the right questions during my clinic visit. This catches some concerns that might have gotten glossed over by the doctor/nurse that I need to better understand.

Another best practice is to engage with support groups. When we interact with other successful patients, we tend to be more successful ourselves.
